1. Rewards.
For the purposes of these Terms, "Reward" is defined as one (1) Credit, Voucher, or Free Trial from one of the following participating entities listed below. Rewards Members can receive up to twelve (12) Rewards in a calendar year; eight (8) Rewards are available to all enrolled customers (“Standard Rewards”), and four (4) Rewards are available to Rewards Members who complete a survey (“Survey Rewards”). The number of Rewards available, Participating Entities, and/or type of Rewards shall be determined by Breezeline and are subject to change at Breezeline’s discretion.
2. Enrollment and Eligibility.
To become a Rewards Member and qualify to redeem Reward(s) under the Program, a customer must enroll in the Breezeline Loyalty Program, accept the Program terms and conditions, and complete a sign up form on www.breezelineRewards.com. Program eligibility validation in the Program may take up to five (5) Business Days to complete. Reward(s) will be available to Rewards Members after Program eligibility validation is complete. Rewards are only available to current Breezeline residential customers who are 18 years of age with a Breezeline account in good standing. To be considered a customer in good standing, a customer is required to pay all Breezeline invoices in full when due for services. By participating in the Program, the Rewards Member agrees that Breezeline may share their name and other information with TLC Worldwide (the “Promoter”), and others, including third-party providers, for the purpose of administering the Program. The Promoter and Breezeline reserve the right to withdraw a customer’s membership to the Loyalty Program at any time.
3. Reward Fulfillment:
a. Standard Rewards
Eight (8) months of the year, on the first Monday of each month, a new Reward will be placed in the Reward platform. Rewards Members will receive an email directing them to the Promoter’s reward platform to redeem the Reward.
b. Survey Rewards
Rewards Members will receive an e mail from Breezeline prompting them to fill out a survey on a quarterly basis. Upon completion of the survey, Rewards Members will receive the Survey Reward via e mail. Survey Rewards will not be available to redeem on the Promoter’s reward platform. Each survey can only be completed one (1) time. Customers can return to and complete the survey at any time during the year. The last day to fill out the survey is 11/1/24.
Each Rewards Member may only redeem each Reward one (1) time. All Rewards Members will only gain access to one (1) Reward per month. If a customer signs up in month one (1), they will have access to one (1) Reward the first month. The second month, they will gain access to a second Reward. If someone signs up in month two (2), they will have access to the Rewards provided in month one (1) and month two (2).
4. General Provisions
The Program will run from 11/1/23 -11/1/24. These dates are subject to change. Rewards Members must redeem all of their Rewards by 11/1/24. Customer may enroll in the Program at any point from 11/1/23-11/1/24 and will be able to redeem all twelve (12) Rewards, subject to the above requirements. All Rewards are subject to promotional availability and may vary by geography, Participating Entity or partner. Promoter reserves the right, in its sole discretion, to replace the Reward with another of equal or higher value. No credit, refund or extension for any unused Rewards will be provided. Rewards have no monetary value. Rewards are not transferable, transferable for sale, resale and is not redeemable for cash. Void if Reward is transferred, sold, auctioned, photocopied or otherwise altered and where prohibited or restricted by law. Reward valid until date indicated.
If a Rewards Member opts out of being in the Loyalty Program, they will lose access to all Rewards at the end of the month in which they opt out. If a Rewards Member opts out and un-enrolls from the Program, then re-signs up to be a member of the Program, they will not receive any of the Rewards they were provided before opting out. If a Rewards Member terminates their Breezeline services, they will lose access to all Rewards immediately upon termination.
Please see the Participating Entities’ Reward or website for additional terms and conditions, which are subject to change at each Participating Entities’ sole discretion. It is the Rewards Member’s sole responsibility to comply with these Terms and Conditions.
The recipient of the Reward must follow the instructions provided by the Promoter, The National Wellness Network, The National Activities Network, The National Entertainment Network, the National Services Network, Stingray Karaoke, Qello Concerts, Readly, Upskillist, Mindfulness app, GoGamers, America’s Test Kitchen, Betterhelp, Classpass and Breezeline. Failure to comply with the instructions or any other directives made in conjunction with this Reward may result in cancellation and/or a charge for the Reward. Reward shall be considered redeemed and participant shall be solely responsible for any costs or fees incurred as a result in cancellation and/or a charge for the Reward. Under these circumstances, Reward shall be considered redeemed and participant shall be solely responsible for any costs or fees incurred as a result of their noncompliance.
Reward code may not cover the full value of the Reward. Additional costs/fees for products and services outside of the Reward are not included and any such fees are the sole responsibility of the Rewards Member. Valid credit card or PayPal account will be required to pay any amounts that exceed the reward amount Reward amount. Credit card may be required to participate in free trial. Reward may auto-renew at the end of the free trial at the selected membership tier. Breezeline and the Promoter cannot be held responsible for any disagreements concerning availability and quality of experience and cannot accept any liability for personal loss or injury that occurs in connection with any Reward. Any dispute must be settled directly with the Participating Entity concerned.
Released Parties shall not be liable for lost, stolen, damaged, misdirected or undeliverable Reward Codes and will not issue or replace Rewards. For the purpose of this Loyalty Program, "Released Parties" includes the Promoter, The National Wellness Network, The National Activities Network, The National Entertainment Network, the National Services Network, Stingray Karaoke, Qello Concerts, Readly, Upskillist, Mindfulness App, GoGamers, America’s Test Kitchen, Betterhelp, Classpass and Breezeline, and their respective parents, affiliates, partners, subsidiaries, agents, distributors and vendors.
Breezeline and the Promoter reserves the right to change or modify these Terms of the Program at any time during the term thereof. By participating in this program you must agree to the Breezeline Privacy Policy.
Only valid to legal residents of Connecticut, Delaware, Florida, Maine, Maryland, New Hampshire, New York, Pennsylvania, South Carolina, Virginia, West Virginia, and Ohio. Void where prohibited or restricted by law.
Rewards are issued by the Promoter located at 60 Hudson Street, Suite 1809, New York, NY. 10013, or at such other address as the Promoter may advise in writing. No inquiry or notice will be validly received by the Promoter unless mailed to the Promoter's address above by first-class mail, postage prepaid.
*Must be 18 years of age. Must be a legal Resident of the United States. Must be a Breezeline Customer in Good Standing. Customer reward bank will receive one (1) reward per month for eight (8) months during the promotion. Customer will also get the opportunity to receive four (4) Rewards by email by completing surveys. All customers who join the program during the promotional period will receive the opportunity to redeem all rewards. Valid credit card or PayPal account may be required for remaining balance of Reward. You may be responsible for paying taxes and fees. Reward may only be available to new customers only. Value of the reward may not cover the full cost of the experience. Each venue has its own terms and conditions you must agree to in order to use the reward. Reward codes expires on 1/31/2024. Void where prohibited and available while supplies last. Breezeline Customer Rewards Promotion contains links to external sites or other online services that are not controlled by Breezeline. Breezeline is not responsible for the privacy practices and data collection policies for such third-party services.
